This is a fully remote position, open to applicants in Ohio.
• Oversee the logistics for patient transfers, including acceptance, consultation, access, and bed placement.
• Determine the suitable level of care, accepting provider, and destination plan for referred patients.
• Collaborate with clinicians and providers to relay precise clinical presentations and patient information.
• Guarantee timely access, suitable bed assignments, accurate documentation, as well as quality, safety, and efficiency.
• Utilize EHR, telephony, and office systems while ensuring the protection of PHI.
• Employ problem-solving, critical thinking, and clinical decision-making skills to recommend optimal patient outcomes.
• Serve as a liaison among patients, physicians, hospital personnel, referring facilities, receiving facilities, and external agencies.
• Monitor compliance and performance for individuals and teams.
• Facilitate process changes and support regulatory and quality initiatives.
• Identify, assess trends, and report obstacles to patient access and throughput.
• Communicate safety, compliance, and operational risks through the established chain of command.
• Engage in process improvement, professional growth, peer development, peer review, and shared leadership.
• Assist in the collection of quality data.
• Maintain an active RN licensure in the states where transfer center services are offered.
• Commit to assigned shifts, including every other weekend, on-call duties, and rotating holidays.
• Execute additional tasks and analyses as requested or directed.
• An active Registered Nurse (RN) license is mandatory.
• An active multistate/compact RN license is required if applicable based on the state of residence.
• An Associate Degree in Nursing or diploma (ADN) is required.
• A minimum of 1 year of acute care experience is required.
• A designated workspace with a locked door is necessary, in compliance with HIPAA regulations.
• A secondary work location is encouraged, adhering to HIPAA regulations.
• A minimum internet speed of 100 Mbps download and 20 Mbps upload is required at both primary and secondary work locations.
• Proficiency in Microsoft Office tools, including Outlook, MS Teams, Excel, and Word is essential.
• Competence in EHR data entry, telephony systems, and office systems is required.
• Familiarity with EMTALA, Ethics and Compliance, quality initiatives, and HIPAA regulations is necessary.
• Ability to review and interpret patient transfer case information is vital.
• Capability to escalate and inform relevant parties about changes in patient status is crucial.
• Proficiency in documenting tasks and discussions related to patient transfers is required.
• Strong critical thinking and clinical decision-making skills are essential.
• Excellent oral and written communication abilities are necessary.
• Skills in active listening, teamwork, relationship building, multitasking, attention to detail, and adaptability are important.
• Experience with the Epic electronic health record system is preferred.
• Genesys contact center experience is preferred.
• Experience in a healthcare contact center is preferred.
• A Bachelor's degree in nursing (BSN) is preferred.
• Over 3 years of acute care experience is preferred.
