Registered Nurse, RN Case Manager

This is a fully remote position, open to applicants in Arizona, +2 more states.

📋 Description

• Operate independently while collaborating with all members of the healthcare team.

• Organize and facilitate care that is both high-quality and cost-effective for patients with ESRD and CKD.

• Reduce fragmentation within the healthcare delivery system.

• Coordinate care among patients, families, physicians, providers, care teams, the community, and the Regional Operations Manager.

• Offer telephonic, field-based, or a combination of both telephonic and field case management.

• Conduct national patient outreach initiatives.

• Assist in preventing complications, minimizing avoidable hospitalizations, and enhancing the overall health of patients with kidney disease.


⛳️ Requirements

• An A.D.N degree from an accredited nursing school is required.

• A current Registered Nurse (RN) license in the state of practice is required.

• A Compact License is preferred.

• MST or PST experience is preferred.

• A willingness to adjust work hours as necessary for patient outreach is required.

• At least 1 year of IKC RN experience is required.

• Ability to work a Monday through Friday schedule is essential.

• Must be able to work independently.

• Continuing education credits must be maintained as required by the state of practice.

• A minimum of three (3) years of clinical nursing experience is required.

• A minimum of one (1) year of experience in renal nursing or case management is preferred.

• Proven knowledge and understanding of data management related to clinical, financial, and patient satisfaction outcomes.

• Demonstrated effectiveness in a change agent role.

• Familiarity with Continuous Quality Improvement (CQI) techniques is required.

• Current CPR certification is mandatory.

• Ability to adapt to new electronic processes and increased productivity expectations is necessary.

• Basic computer skills and proficiency in MS Word and Outlook are required.

• Functional proficiency with DaVita-specific clinical software programs, including Capella, is required within 90 days of employment.

• A home office with internet connectivity of at least 1MB upload and 1MB download speed is required.
