Registered Nurse, Ohio

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Administer a variety of assessments and screening tools throughout the care continuum.

• Analyze assessment and screening results to prioritize the needs of members.

• Create personalized care plans in collaboration with the multidisciplinary care team.

• Facilitate internal and joint rounds to guarantee a seamless and coordinated experience for members.

• Acquire knowledge and utilize Tuesday Health electronic systems, tools, and technology.

• Foster strong, trusting relationships with members by actively listening and demonstrating empathy.

• Act as the central coordinator of the multidisciplinary team for members facing serious illnesses.

• Provide thorough, person-centered care to individuals with significant health challenges.

• Lead efforts in care coordination and collaboration across the care team.

⛳️ Requirements

• Must reside in Ohio.

• Hold an active and unrestricted Registered Nurse license in the State of Ohio with no board actions.

• Previous experience in a clinical or medical setting is preferred.

• Familiarity with a multidisciplinary care model, engaging with network providers, health plan care management, and employed clinicians.

• Excellent communication and organizational skills.

• Capability to multitask and perform under pressure while maintaining a positive experience for members.

• Ability to recognize when to escalate issues and respond professionally in emergency situations.

• Strong technical proficiency to navigate various documentation systems and tools; willing to learn new systems as the organization develops.

• Strong writing abilities.

• Capacity to drive during the day, at night, or in adverse weather conditions.

• Valid driver’s license with a clean driving record.

• Minimum state-required automobile insurance coverage.
