This is a fully remote position, open to applicants in Massachusetts.

📋 Description

• Administer assessments and screening tools throughout the care process.

• Analyze assessment results to prioritize the needs of members.

• Create personalized care plans in collaboration with the multidisciplinary care team.

• Facilitate internal and joint rounds to enhance and coordinate the member experience.

• Utilize Tuesday Health's electronic systems, tools, and technology to provide coordinated care.

• Foster strong, trusting relationships with members through active listening and empathy.

• Act as the primary coordinator for the multidisciplinary team for members facing serious illnesses.

• Provide comprehensive, person-centered care to individuals dealing with serious health conditions.

• Promote collaboration and enhance the member experience.

⛳️ Requirements

• Hold an active and unrestricted Registered Nurse license in Massachusetts with no board actions.

• Preferred experience in a clinical or medical environment.

• Familiarity with a multidisciplinary care model involving network providers, health plan care management, and employed clinicians.

• Excellent communication and organizational skills.

• Capability to multitask and perform under pressure while maintaining a positive member experience.

• Ability to determine when to escalate issues and maintain professionalism in urgent situations.

• Proficient technical skills to navigate documentation systems and tools.

• Willingness to learn new systems as the company grows and evolves.

• Strong writing abilities.

• Capability to drive during the day, night, or in inclement weather conditions.

• Possess a valid driver's license with a good driving record.

• Meet state minimum automobile insurance requirements.
