Registered Nurse – Home Health

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Deliver exceptional home health care to patients in their preferred living environments.

• Ensure all activities align with Elara Caring’s vision and Home Health leadership.

• Uphold patient and staff privacy and confidentiality in accordance with HIPAA regulations.

• Evaluate assigned home health patient caseloads and families for their physical, psychosocial, and environmental needs.

• Take primary responsibility for caseloads, which includes assessing, planning, coordinating, implementing, and evaluating treatment plans.

• Gather physical, psychological, social, and spiritual patient information and integrate it into care plans.

• Perform initial assessments and reassess nursing needs during visits.

• Conduct follow-up visits based on the requirements of patients, families, and facilities.

• Relay significant findings, issues, changes in health conditions, environmental concerns, and unsafe facility conditions to appropriate clinical and care staff.

• Update care plans in response to patient-care challenges and inform the Clinical Supervisor, physician, facility, and team members.

• Report to the Clinical Manager or Clinical Supervisor.

• Travel within the branch/office coverage area and between patient homes and assigned work locations.


⛳️ Requirements

• Valid, unrestricted state RN license.

• Over 1 year of nursing experience in a clinical care environment.

• Willingness and ability to travel within the branch/office coverage area.

• Capability to frequently lift, push, pull, and support up to 50 pounds, including the positioning or transferring of patients and moving equipment.

• Possession of a valid driver's license with an acceptable driving record, or reliable transportation between patient homes and assigned work sites.

• Willingness and ability to travel 50% of the time.

• Preferred: Associate’s Degree or Bachelor’s Degree in Nursing.

• Preferred: Previous experience in Home Health or Hospice settings.

• Preferred: Familiarity with HomeCare HomeBase.

• Must adhere to maintaining patient and staff privacy and confidentiality per HIPAA Privacy Final Rule.
