Nurse Practitioner – National After-Hours Team, Part Time

This is a fully remote position, open to applicants in New York, +1 more state.

📋 Description

• Deliver compassionate telephonic care and clinical guidance during evenings, nights, weekends, and holidays.

• Act as the primary support for residents and facility staff during after-hours.

• Offer medical direction and care via telephone.

• Utilize Curana’s telephonic platform for making and receiving calls.

• Coordinate care among facilities, hospitals, and clinics.

• Provide high-quality, cost-effective care for acute, chronic, and behavioral health issues in partnership with physicians and specialty providers.

• Conduct comprehensive assessments.

• Accurately and thoroughly document encounters in the EMR in compliance with CMS regulations.

• Implement Curana’s clinical protocols and practice guidelines to ensure safe treatment in place whenever feasible.

• Conduct at least 30% of visits via telehealth, which includes evaluations of acute changes, falls, and controlled substance visits.

• Engage in mandatory education and training sessions.

• Cover shifts during weeknights, weekends, overnight, and holidays.

• Provide coverage for skilled nursing and senior living facilities across various states.

⛳️ Requirements

• Master’s Degree in Nursing as a Nurse Practitioner.

• Current unrestricted NP license in your state of residence.

• Ability to secure NP licensure in other required states within 60 days.

• National certification as a Nurse Practitioner (ANP, FNP, or GNP).

• Capacity to obtain DEA licensure / Prescriptive Authority.

• Experience in managing acute and chronic diseases.

• Clinical experience in adult, family, or geriatric care.

• A minimum of 3 years of experience as a Nurse Practitioner.

• Ability to establish a collaborative practice agreement, if applicable in your state(s).

• Willingness to work scheduled shifts according to scheduling policies.

• Proficient computer skills, including the capability to document medical information using written and electronic medical records.

• Background in working in nursing homes or with seniors in acute care settings.

• Knowledge of geriatrics, chronic illnesses, and acute disease management.

• Familiarity with advanced illness and end-of-life discussions.

• Skill in developing and maintaining positive customer relationships.

• Ability to adapt to change.

• Availability for overnight shifts and holidays is essential.

• Part-time employees are required to work two holidays per year.

• Must handle high call volumes and conduct at least 30% of visits via telehealth.

• Capability to manage multiple calls independently while providing care across several states.
