Registered Nurse Case Manager – Field

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Assist members across Essex County and surrounding regions.

• Perform in-home, direct assessments for individuals enrolled in Managed Long-Term Services and Supports and Dual Special Needs Programs.

• Assess member requirements and facilitate adult/pediatric medical day care, personal care assistant services, nursing facility placements, and MLTSS program enrollments.

• Collaborate with members, caregivers, primary care providers, and interdisciplinary teams to design and execute tailored care plans.

• Approve services in accordance with program regulations.

• Aid in ensuring safe transitions of care.

• Accurately and promptly record all case activities in the electronic health record.

• Utilize clinical judgment and critical thinking to address complex care needs.

• Provide mentorship to new team members upon achieving proficiency.

⛳️ Requirements

• Current, unrestricted RN license in New Jersey.

• Comfort and experience conducting field-based home visits across diverse socioeconomic levels and community environments, including various neighborhoods and situations.

• Over 3 years of clinical practice experience.

• More than 3 years of experience in home health, hospice, or comparable settings.

• Capability to manage pediatric and medically intricate cases.

• Residency in Essex County, NJ.

• Successful completion of the NJ Choice Certification, which includes achieving a minimum exam score of 80%, finishing state training modules, and completing a field mentoring component.

• Dependable transportation.

• Willingness and ability to travel up to 75% of the time.

• At least an Associate Degree in nursing.

• Experience with LTSS is preferred.

• Background in case management and/or discharge planning is preferred.

• Managed care experience is preferred.

• Familiarity with Medicaid and Medicare is preferred.

• Proficiency in using various systems and the MS Office Suite (Outlook, Word, Excel, SharePoint, Teams) is preferred.
