
Registered Nurse β Case Manager

This is a fully remote position, open to applicants in Pennsylvania.
β’ Evaluate, plan, implement, coordinate, supervise, and assess options and services to enhance patient or member health outcomes.
β’ Oversee utilization and practice metrics to improve care delivery and optimize clinical, quality, and financial results.
β’ Incorporate evidence-based clinical guidelines, preventive measures, protocols, and other metrics into patient-focused treatment strategies.
β’ Create care systems that track progress and encourage early intervention in acute care scenarios.
β’ Support the design, execution, and assessment of the advanced patient-centered care model.
β’ Evaluate the healthcare, educational, and psychosocial requirements of patients or members.
β’ Develop personalized care plans and promote collaboration among patients or members, families, primary care providers, and healthcare teams.
β’ Analyze laboratory results, diagnostic tests, utilization trends, and other metrics to evaluate quality and efficiency.
β’ Utilize benefits and management resources to assist patients or members and healthcare teams.
β’ Keep necessary documentation for case management activities organized.
β’ Gather essential data and apply it to modify treatment plans as necessary.
β’ Collaborate with a dedicated team to provide patient-centered care coordination and achieve outstanding outcomes.
β’ Minimum of two (2) years of experience as a Registered Nurse (RN).
β’ Graduate from a Specialty Training Program in Nursing.
β’ Licensed Registered Nurse in Pennsylvania (RN_State of Pennsylvania).
β’ Familiarity with healthcare laws, regulations, and insurance coverage policies is advantageous.
β’ Competence in healthcare IT systems and insurance claims processes is beneficial.
β’ Ability to function independently and as part of a multidisciplinary team is advantageous.
β’ Certification as a Case Manager by the American Case Management Association (ACMA) is preferred.
β’ Bachelor of Science in Nursing (BSN) is preferred.
β’ Experience in case management is preferred.
