Referral Navigator

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee EMR/referral portals in alignment with Referral Acceptance Guidelines

• Respond promptly to referral requests

• Utilize knowledge and critical thinking skills when accepting referrals

• Maintain professional and appreciative communication with referral sources

• Confidently articulate the agency’s services

• Perform patient insurance pre-checks as necessary

• Forward referrals needing clinical approval to the relevant operations leader

• Attach and scan referral packets and insurance pre-check documentation into the intake system

• Collaborate with the sales team to ensure a smooth transition in care for both referral sources and patients

• Expertise in managing EMR portals and adhering to acceptance guidelines

• Strong communication skills and a collaborative work style

• Detail-oriented with proficient communication abilities

• Competent and precise documentation practices

• Patience, adaptability, and a team-oriented mindset

• Exceptional customer service skills

• Familiarity with medical terminology is preferred

• Accurate data entry capabilities

• Ability to work both independently and as part of a team

• Proficient in Microsoft Office Suite, CRM software, and Home Care Home Base software

• Associate’s degree is preferred

• Minimum of one year of experience in home health preferred

• At least one year of experience in a customer service role

• Successful completion of a pre-employment drug screening

• Legally authorized to work in the United States
