Practical/Vocational Nurse

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver direct patient care in accordance with the state's Nurse Practice Act.

• Execute and adhere to personalized care plans established by the RN.

• Educate patients and their families on self-care methods and diagnosis-related information.

• Accurately document care in a timely manner.

• Collaborate with the care team, including OT, PT, HHA, and other disciplines, while communicating patient needs to the RN or Clinical Supervisor.

• Utilize supplies and equipment safely, following aseptic techniques.

• Perform additional duties as necessary to support patient services.

⛳️ Requirements

• Graduate from an accredited practical or vocational nursing program.

• Hold an active LPN or LVN license in California.

• Possess 3+ years of nursing experience, with a preference for home health or med/surg background.

• Current CPR certification from the AHA.

• Valid driver’s license, a reliable insured vehicle, and the capability to travel to patient homes.

• Strong communication skills along with a collaborative, patient-centered approach.
