Post-Acute Care Admission Coordinator, Part-time

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Manage pre-screening referrals, the patient admission process, and PPS for the Acute Rehabilitation Center.

• Oversee the admission, referral, and PPS systems for the Acute Rehabilitation unit.

• Provide support during periods of staff shortages.

• Contribute to the training of staff members.

⛳️ Requirements

• Associate's Degree in Nursing.

• Valid Illinois Registered Nurse License.

• A minimum of 5 years of technical experience.

• BLS certification is preferred.
