Population Health Pharmacy Technician I, Spanish Speaking

This is a fully remote position, open to applicants in Texas.

📋 Description

• Assist in the coordination, implementation, and monitoring of pharmaceutical services designed to enhance health outcomes within a community or population.

• Provide support for medication reconciliation processes for patients.

• Work in partnership with pharmacists and healthcare providers to optimize medication regimens.

• Evaluate patient profiles to identify medication-related issues and discrepancies.

• Gather and analyze medication-related data to uncover trends and opportunities for improvement.

• Produce reports on medication adherence, therapeutic outcomes, and other relevant metrics.

• Coordinate pharmaceutical services alongside healthcare teams.

• Engage with patients, caregivers, and healthcare professionals regarding medication-related concerns.

• Take part in population health programs that promote medication adherence and preventive care.

• Support initiatives aimed at enhancing health outcomes and minimizing medication-related disparities.

• Ensure adherence to regulatory standards, pharmacy laws, and organizational policies.

• Contribute to quality improvement efforts within the pharmacy.

⛳️ Requirements

• High school diploma or its equivalent.

• Preferred completion of a pharmacy technician program.

• Active licensure as a pharmacy technician in Texas and the state of residence.

• Strong grasp of pharmacy operations, medication management, and healthcare systems.

• Excellent communication, interpersonal, and organizational abilities.

• Proficient in using pharmacy software and computer systems for data entry and analysis.

• Ability to collaborate effectively in a multidisciplinary team environment.

• Fluency in Spanish is preferred.
