Population Health Pharmacy Technician I

This is a fully remote position, open to applicants in Texas.

📋 Description

• Assist in the coordination, implementation, and monitoring of pharmaceutical services designed to enhance health outcomes within a specific population or community.

• Support the processes of medication reconciliation.

• Collaborate with pharmacists and healthcare providers to optimize medication regimens.

• Review patient profiles to identify any medication-related issues and discrepancies.

• Collect and analyze medication-related data to uncover trends and opportunities for improvement.

• Generate reports concerning medication adherence, therapeutic outcomes, and pertinent metrics.

• Coordinate pharmaceutical services alongside healthcare teams.

• Communicate effectively with patients, caregivers, and healthcare professionals regarding medication-related concerns.

• Participate in population health programs that promote medication adherence and preventive care.

• Support initiatives aimed at enhancing health outcomes and minimizing medication-related disparities.

• Ensure compliance with regulatory standards, pharmacy laws, and organizational policies.

• Contribute to quality improvement efforts within the pharmacy.

⛳️ Requirements

• High school diploma or its equivalent.

• Completion of a pharmacy technician program is preferred.

• Active licensure as a pharmacy technician in Texas and in the state of residence.

• Strong understanding of pharmacy operations, medication management, and healthcare systems.

• Excellent communication, interpersonal, and organizational abilities.

• Proficient in using pharmacy software and computer systems for data entry and analysis.

• Ability to collaborate effectively in a multidisciplinary team environment.

• Fluency in Spanish is preferred.
