This is a fully remote position, open to applicants in United States.
• Design, implement, and assess population health initiatives focused on medication management, adherence, and enhancing therapeutic outcomes.
• Leverage health data analytics to pinpoint trends, identify care gaps, and discover intervention opportunities aimed at improving medication-related outcomes for the designated population.
• Partner with healthcare providers to guarantee appropriate medication choices, dosing, and monitoring, taking into account population-specific needs and evidence-based guidelines.
• Create and execute programs to educate patients regarding their medications, ensuring comprehension, adherence, and management of potential side effects.
• Collaborate closely with physicians, nurses, social workers, and other healthcare professionals to synchronize care plans and interventions that lead to enhanced patient outcomes.
• Engage in quality improvement projects focused on advancing medication-related outcomes, minimizing adverse events, and endorsing evidence-based practices.
• Keep abreast of pharmaceutical regulations and ensure adherence to relevant laws, policies, and guidelines.
• Provide leadership within the pharmacy team, mentor junior staff, and support departmental goals and initiatives.
• Doctor of Pharmacy (PharmD) degree from an accredited institution.
• Active pharmacist licensure in both your state of residence and Texas in good standing.
• Strong analytical skills and the ability to interpret data effectively.
• Experience in pharmacy practice, ideally within a population health or managed care environment.
• Exceptional communication, collaboration, and leadership skills.
• Familiarity with healthcare regulations, policies, and quality improvement methodologies.
