POD Pharmacist

This is a fully remote position, open to applicants in Texas.

📋 Description

• Perform and document detailed patient-specific comprehensive medication reviews (CMRs)

• Propel medication interventions that assist providers and patients in adhering to evidence-based medication guidelines

• Assess treatment plans, clinical documentation, and medical records

• Create medication action plans and facilitate targeted clinical outreach initiatives

• Support quality assurance processes

• Conduct telephonic consultations with treating providers when medication modifications are clinically necessary

• Engage with patients to enhance medication usage, adherence, and understanding of drug therapy

• Act as a pharmacy resource for field clinicians regarding medication use and optimization in complex polychronic conditions

• Provide medication optimization training to Monogram clinicians

• Monitor trends in medication availability and usage

• Maintain up-to-date knowledge of evidence-based prescribing practices for cardiovascular, endocrine, and kidney diseases

• Participate in interdisciplinary case rounds and meetings

• Create medication-related documentation for treating providers

• Execute medication optimization processes for partners and patients

• Assist in delivering personalized physical and mental health care and perform additional assigned tasks

⛳️ Requirements

• Bachelor’s degree in Pharmacy or Doctor of Pharmacy (PharmD)

• Current, unrestricted pharmacist license in the state of service

• A minimum of three (3) years of active pharmacist practice experience

• Experience ideally in a managed care, ambulatory care, or clinical patient care setting

• Preferred PGY1 residency in clinical fields

• Board certification in Medication Management or a related clinical field is preferred

• MTM certificate training from APhA, PowerPak, or a similar program is preferred

• Capability to work from a professional, remote location in the assigned area of service

• Availability from Monday to Friday, 8:00 AM to 5:00 PM local time, and as required by business needs

• Some domestic travel may be required
