This is a fully remote position, open to applicants in Texas.

📋 Description

• Provide expert PTA visits in patients’ residences following the supervising PT’s care plan.

• Implement treatment strategies aimed at enhancing functional mobility and tracking measurable progress, including gait/balance, transfers, strengthening, home safety, and fall prevention.

• Establish and monitor objective progress within the PTA's scope while relaying updates and concerns to the supervising PT.

• Collaborate with the multidisciplinary care team by addressing changes in patient conditions and aligning on care directives.

• Guide patients and their caregivers on effective home programs.

• Accurately and promptly document in Homecare Homebase (HCHB).


⛳️ Requirements

• Valid Texas PTA license in good standing.

• Strong clinical judgment and the ability to work independently in the field within PTA scope and under PT supervision.

• Capacity to report changes in patient conditions to PT/RN/MD and document the rationale clearly.

• Excellent communication and documentation skills.

• Consistent follow-through on treatment plans, visit schedules, and patient objectives.

• Valid driver’s license and willingness to travel daily within the assigned territory.

• Ability to carry and transport basic therapy equipment as necessary.

• Proficiency in core home health PTA responsibilities.

• Safety-first mobility skills, which include transfers, gait training, balance/fall prevention, and assistive device education.

• Knowledge in strengthening, functional mobility, and home safety assessment/modification education.

• Experience in coaching patients/caregivers on practical home programs.

• Previous home health experience is preferred.

• Familiarity with Homecare Homebase (HCHB) or other similar home health EMR is preferred.

• Advanced clinical experience in high-signal ortho/neuro populations is preferred.

• Strong organizational and time management skills are preferred.

• Willingness to undergo a background check and drug screening.

• Legal authorization to work in the country of application.
