This is a fully remote position, open to applicants in Texas.
• Provide professional PTA visits in patients' residences according to the supervising PT's care plan.
• Implement treatment strategies aimed at enhancing functional mobility and achieving measurable progress, including gait/balance, transfers, strengthening, home safety, and fall prevention.
• Establish and monitor objective progress within the PTA's scope of practice.
• Relay updates and concerns to the supervising PT.
• Collaborate with the multidisciplinary care team and address any changes in patient condition.
• Ensure orders are aligned and maintain effective care coordination.
• Mentor patients and their caregivers on practical home exercise programs.
• Document interactions promptly and comprehensively in Homecare Homebase (HCHB).
• Restore patient functionality, promote safety at home, and enhance measurable outcomes.
• A valid Texas PTA license (in good standing).
• Strong clinical judgment and the ability to operate independently in the field while adhering to PTA guidelines and PT supervision.
• Capability to report changes in patient condition to PT/RN/MD and articulate the rationale clearly.
• Excellent communication and documentation skills.
• Consistent follow-through on treatment plans, visit schedules, and patient objectives.
• Valid driver's license and willingness to travel daily within the assigned area.
• Ability to carry and transport essential therapy equipment as necessary.
• Proficient in core home health PTA responsibilities.
• Expertise in transfers, gait training, balance/fall prevention, and education on assistive devices.
• Knowledge in strengthening, functional mobility, and home safety assessment/modification education.
• Proficient in coaching patients and caregivers on practical home programs.
• Previous home health experience is preferred.
• Familiarity with Homecare Homebase (HCHB) or similar home health electronic medical records (EMR) systems is preferred.
• Advanced clinical experience with high-signal orthopedic/neuro populations is preferred.
• Strong organizational and time management skills are preferred.
• Willingness to undergo a background check and drug screening.
• Legal authorization to work in the country of application.
• Compensation of $65 per visit for regular PT sessions.
