Pharmacy Care Coordinator I

This is a fully remote position, open to applicants in United States.

📋 Description

• Assist in optimizing medication management, allowing physicians to concentrate on patient care while providing support for care coordination for designated patient groups.

• Aid in the fulfillment of patient orders to ensure timely receipt of medications.

• Conduct telephonic follow-up for medication management with patients suffering from chronic conditions.

• Organize the delivery of medications to patients.

• Support patients' clinical requirements by collaborating with the Pharmacist.

• Oversee patient orders through the Pharmacist's verification process.

• Execute medication management service tasks and maintain documentation within Care Management systems.

• Access, enter, and retrieve information from the pharmacy network to ensure precise records.

• Secure refill authorizations from prescribing professionals.

• Prepare and process claims for third-party insurance.

• Keep patient/client profiles up to date and accurate.

• Professionally handle phone calls and direct them as necessary.

• Accept prescriptions for processing and confirm that the PIC has the required order details.

• Address customer inquiries and escalate clinical matters to the Pharmacist.

• Perform additional related duties as assigned.

⛳️ Requirements

• High school diploma or its equivalent.

• A pharmacy technician license or pharmacy technician trainee license issued by the Texas State Board and the state of residence.

• Excellent communication and customer service abilities.

• Flexibility to work varied hours, including evenings and weekends.

• Capability to read and accurately transcribe pharmaceutical information.

• Ability to remain seated for extended periods and stand intermittently.

• Compliance with HIPAA and relevant federal and state confidentiality regulations is essential.

• Adherence to established procedures, rules, and regulations is required.
