Pharmacist, Home Infusion

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Interpret and assess physician medication orders.

• Precisely prepare, calculate, and dispense medications in compliance with Pennsylvania and federal regulations.

• Detect and report any suspected adverse drug reactions.

• Assist in formulary management and therapeutic interchange protocols.

• Review and maintain patient medication records, ensuring consideration for allergies, interactions, and therapeutic duplications.

• Coordinate daily pharmacy operations and oversee pharmacy technicians, aides, interns, and other staff.

• Ensure adherence to regulatory requirements, professional standards, and hospital quality policies.

• Communicate effectively with physicians, nurses, and care teams regarding medication orders and care decisions.

• Provide drug information, education, and support to healthcare professionals and pharmacy students.

• Utilize pharmacy information systems and clinical databases.

• Process and validate prescription orders.

• Participate in rotating shifts, on-call responsibilities, weekends, evenings, and holidays.

⛳️ Requirements

• Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited institution.

• Valid Pennsylvania pharmacist license.

• A minimum of 3 months of experience in a hospital pharmacy setting (preferred).

• Familiarity or experience with home infusion and IV admixture preparation (preferred).

• For remote work, a cable broadband or fiber optic internet connection with at least 10Mbps download and 5Mbps upload speed.

• Willingness to participate in rotating shifts, including evenings, weekends, on-call responsibilities, and holidays.
