Patient Care Coordinator

This is a fully remote position, open to applicants in Canada.

📋 Description

• Act as the initial point of contact for patients and their families contacting Sailor Health.

• Assist older adults and caregivers through the intake procedure.

• Offer patient support, care coordination, and emotional assistance.

• Aid patients in accessing care by connecting them with the appropriate therapist.

• Provide ongoing support to patients throughout the intake and care experience.

• Help enhance care access for older adults and their families.

• Take initiative and contribute to shaping essential processes within a small, agile team.

⛳️ Requirements

• Must reside in Canada.

• An empathetic and clear communicator.

• Highly organized with strong attention to detail.

• Comfortable with learning Google Workspace, EHR platforms like Healthie, and CRMs such as HubSpot.

• Experience in healthcare, customer service, or administrative support is beneficial but not mandatory.

• Interest in mental health and improving healthcare access.

• Comfortable working hours in the Eastern Time Zone.

• Willingness to work one of the specified shifts: 9a–6p ET or 12p–9p ET.
