Patient Care Coordinator

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Manage monthly patient refill requests and ensure the accuracy of documentation prior to dispatching orders.

• Process orders for new patients and maintain electronic medical records.

• Collaborate with the patient admissions team to facilitate patient onboarding for services.

• Respond to incoming inquiries from patients, caregivers, branches, and patient referral sources about the status of nutrition therapy orders.

• Handle prescription modifications.

• Direct calls to appropriate departments.

• Confirm patient demographics and insurance details to guarantee precise shipment.

• Begin the referral process and address inquiries from patients and caregivers regarding admissions status.

⛳️ Requirements

• At least one year of experience in a customer service role.

• Proficient in data entry.

• Familiarity with Microsoft Office, particularly Excel, Outlook, and Word.

• High School diploma or GED required.

• Preferred experience in home infusion or durable medical equipment (DME).

• Preferred experience in a call center environment.

• Preferred background in a healthcare setting.

• Candidates must reside in the United States.
