Patient Care Associate

This is a fully remote position, open to applicants in Canada.

📋 Description

• Act as the primary point of contact for patients and their families reaching out to Sailor Health.

• Assist older adults and caregivers throughout the intake procedure.

• Deliver warm, clear, and professional support to patients.

• Organize care and assist patients in connecting with the appropriate therapist.

• Offer emotional support and reassurance during the intake experience.

• Ensure patients feel supported, understood, and confident as they begin their care journey.

• Oversee multiple patients at various stages of the intake process.

• Take initiative and contribute to shaping essential processes within a small, dynamic team.

⛳️ Requirements

• Located in Canada.

• Strong empathy along with excellent communication abilities.

• Proficient in active listening and capable of managing challenging or emotional discussions.

• Highly organized and attentive to detail.

• Capable of handling multiple patients at different phases of the process.

• Comfortable with learning Google Workspace, EHR systems like Healthie, and CRMs such as HubSpot.

• Quick to adapt to new software applications.

• An interest in mental health and improving healthcare accessibility.

• Experience in healthcare, customer service, or administrative support is advantageous but not mandatory.

• Willingness to work hours aligned with the Eastern Time Zone.

• Availability to work during one of the specified shifts: either 9 a.m.–6 p.m. ET or 12 p.m.–9 p.m. ET.
