Part-Time LPN Pre-Admission Coordinator – Home Health

This is a fully remote position, open to applicants in Massachusetts.

📋 Description

• Evaluate incoming referrals to ensure clinical documentation is complete and accurate.

• Monitor patients who are awaiting discharge or surgery and follow up on any outstanding items.

• Verify physician information, orders, face-to-face documentation, and other pre-admission details.

• Collaborate with intake, sales, referring facilities, and clinical teams to address any missing information.

• Follow up on discharge orders and documentation necessary to advance referrals.

• Conduct patient welcome calls or confirm that they have been completed.

• Record updates and maintain referral workflows in Homecare Homebase.

• Assist in ensuring referrals are prepared for scheduling and that care begins promptly.

⛳️ Requirements

• Active LPN license in Massachusetts.

• Experience in skilled home health care and familiarity with the intake and referral processes.

• A minimum of one year of practical nursing experience.

• Exceptional attention to detail and the capability to manage multiple pending referrals simultaneously.

• Willingness to learn and utilize electronic clinical systems effectively.

• Experience with Homecare Homebase is advantageous.
