This is a fully remote position, open to applicants in United States.

📋 Description

• Engage with patients through telephone and/or video to deliver compassionate, evidence-based home-based palliative care.

• Execute care coordination and case management responsibilities, which include following up on DME orders, collaborating with home health professionals, and liaising with primary care providers (PCPs).

• Take part in daily team huddles and weekly palliative rounds alongside the interdisciplinary team.

• Maintain a patient panel and perform regular check-ins based on risk assessment or health status changes.

• Refer patients to the palliative social worker and Nurse Practitioner (NP) as necessary.

• Collaborate with other Devoted Medical teams to ensure integrated care delivery.

• Assist in triaging and responding to incoming calls from palliative patients.

• Promote palliative education initiatives for Devoted Medical staff.

• Work alongside palliative care colleagues to support, care for, and enhance the program and its patients.


⛳️ Requirements

• Unrestricted RN licensure, with a compact license preferred.

• Capability to obtain licensure in additional states if required within 90 days of the hire date.

• At least 5 years of RN experience, including a minimum of 2 years in hospice or palliative care settings.

• CHPN® certification, or the willingness to pursue it within the first six months of employment.

• Flexibility to work across different time zones.

• Proficiency with technology and comfort in a fast-paced work environment.

• Self-motivated individual with the ability to thrive in a work-from-home setting.

• Strong interpersonal and communication abilities.

• Active Basic Life Support (BLS) certification.

• ELNEC® or EPEC® training is preferred.

• Bilingual proficiency in English and either Creole, Spanish, or Vietnamese is preferred.

• Previous nursing experience in ICU, oncology, or geriatrics is preferred.

• Prior inpatient nursing experience is preferred.

• Experience in program development is preferred.
