Outreach Specialist

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Connect virtually with members who qualify for Cityblock services, engaging them based on their needs.

• Perform outreach and engagement through telephonic and virtual means.

• Utilize Commons data to gain insights about potential members and clarify outreach procedures.

• Assess demographic details and accurately document information along with emergency contacts in Commons.

• Gather both verbal and written consent.

• Start the intake process and transition members to care teams.

• Relay urgent needs and relevant details to the care teams.

• Monitor tasks, evaluate performance, and maintain communication with care teams, members, and family members within Commons.

• Offer feedback for product development and assist in onboarding as a prospective super-user.

⛳️ Requirements

• Minimum of 1 year experience in a comparable position.

• Competence in delivering telephonic and virtual outreach and engagement services.

• Proficiency in using Commons for data collection, information organization, task tracking, and performance monitoring.

• Capability to review and accurately document demographic information and emergency contacts.

• Skill in collecting both verbal and written consent.

• Ability to initiate intake procedures and facilitate handoffs to care teams.
