This is a fully remote position, open to applicants in Rhode Island.
• Facilitate triage, case management, and care navigation for oncology patients with potential comorbidities.
• Provide direct patient care through various channels including phone, email, video, and virtual platforms.
• Offer consultative clinical support and address urgent clinical escalations.
• Triage Symptom Survey responses and manage inbound reported symptoms, assisting patients or escalating to a Nurse Practitioner as necessary.
• Recognize and address needs related to care coordination, nutritional support, discharge planning, and provider referrals.
• Educate patients regarding their diagnosis, Daymark Health’s care services, and subsequent steps.
• Foster trusting relationships with patients and their care networks.
• Coordinate integrated care alongside Nurse Practitioners, Medical Directors, Social Workers, and non-clinical Health Partners.
• Tackle social determinants of health, such as food security and transportation accessibility.
• Adhere to policies, procedures, documentation standards, clinical protocols, and operational processes.
• Collaborate with Clinical Operations to enhance systems, tools, and technology.
• A Bachelor of Science Degree in Nursing is required.
• A compact unrestricted registered nurse (RN) license, or the capability to obtain one.
• Licensure in Rhode Island, or a willingness to secure licensure in Rhode Island.
• Minimum of 3 years nursing experience, with at least 2 years in oncology nursing.
• A strong desire to assist patients in understanding the benefits of palliative care.
• Oncology-related certifications such as OCN, AOCN, AOCNS, or CCM are strongly preferred, or must be obtained within 2 years of hire.
• Proficient in using Slack, Zoom, Google Suite, and EHR systems.
• Ability to build rapport and work effectively with diverse populations.
• Capacity to independently prioritize urgent and important needs.
• Patient-centered approach and dedication to Daymark’s mission.
• Excellent organizational, multitasking, listening, and communication skills.
• Adaptability in dynamic and uncertain environments.
• Basic Life Support (BLS) certification is required for patient-facing roles by the start date.
• Compliance with Daymark’s Employee Vaccination and Testing policy.

• Monday–Friday work schedule from 8:30 AM to 5:00 PM local time.
