On-Call Licensed Practical Nurse

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver direct patient care in accordance with the Nurse Practice Act of the state.

• Execute and adhere to personalized care plans established by the RN.

• Provide education to patients and their families regarding self-care methods and information related to their diagnosis.

• Accurately document care provided in a timely manner.

• Collaborate with the care team, which includes OT, PT, HHA, and other professionals, and communicate patient requirements to the RN or Clinical Supervisor.

• Utilize supplies and equipment safely, following aseptic techniques.

• Carry out additional duties as necessary for patient services.

⛳️ Requirements

• Graduate from an accredited practical or vocational nursing program.

• Hold an active LPN or LVN license in California.

• Possess 3+ years of nursing experience; home health or med/surg experience is preferred.

• Current CPR certification from AHA.

• Have a valid driver’s license, a reliable insured vehicle, and the capacity to travel to patient homes.

• Demonstrate strong communication skills along with a collaborative, patient-centered approach.
