Nurse Practitioner – Weekend On-Call

This is a fully remote position, open to applicants in Iowa.

📋 Description

• Evaluate the medical and mental health requirements of patients.

• Develop proactive care plans to manage conditions and overcome barriers to care.

• Ensure adherence to local, state, and federal regulations regarding clinical services.

• Prescribe appropriate medications.

• Request lab work, diagnostic tests, and consultations as necessary.

• Monitor patients’ adherence to treatment and their response.

• Adjust treatment plans as required.

• Collaborate with RN case managers within an integrated care team.

• Work in partnership with providers, case managers, facility partners, and health plan leadership.

• Implement TruHealth’s nationwide care model in nursing homes or assisted living facilities.

• Offer training and guidance to facility staff as needed.

⛳️ Requirements

• A valid FNP, AGNP, AHACNP, or PA license is mandatory.

• Must hold a degree from an accredited APRN or PA program.

• A minimum of 3 years’ experience in clinical nursing or rehabilitation within geriatric populations is required.

• Proficiency in Electronic Health Records is essential.

• Familiarity with Microsoft applications such as Word, Outlook, and Excel is needed.

• A Master’s degree or higher is preferred.

• Experience in primary care focused on geriatric medicine is advantageous.

• Background in long-term care is preferred.

• APRN Advanced Practice RN certification/license is necessary.

• Must be able to adhere to federal and state regulations.

• Capability to exercise independent judgment and seek consultation with a supervising physician when appropriate.
