Nurse Practitioner, Urgent Care

This is a fully remote position, open to applicants in Florida, +3 more states.

📋 Description

• Perform telehealth assessments for Medicare beneficiaries facing urgent yet non-emergent medical issues.

• Assess symptoms, analyze medical histories, and identify suitable interventions.

• Decide when it is clinically necessary to escalate care to the member’s primary care provider, an emergency department, or 911.

• Accurately and promptly document all interactions in the electronic medical record.

• Aid in minimizing preventable emergency department visits and hospital admissions through proactive clinical intervention.

• Educate patients regarding disease management, medications, lifestyle changes, and symptom monitoring.

• Encourage medication compliance and empower patients to manage chronic conditions independently.

• Offer clinical guidance and training to nursing staff on appropriate escalation protocols.

• Engage in clinical case reviews and contribute to quality improvement efforts.

⛳️ Requirements

• Valid and unrestricted Nurse Practitioner license; Georgia license preferred.

• National certification relevant to the specialty.

• At least 3 years of clinical practice experience, ideally in primary care, internal medicine, family medicine, geriatrics, urgent care, or chronic disease management.

• Proficient in managing hypertension and common chronic illnesses, such as diabetes, heart failure, COPD, and chronic kidney disease.

• Previous experience in telehealth or virtual care settings.

• Strong clinical evaluation and decision-making capabilities.

• Exceptional verbal communication skills and patient education proficiency.

• Knowledge of eClinicalWorks (eCW) or comparable electronic medical records systems.
