This is a fully remote position, open to applicants in Texas.
• Act as the main clinical contact for patients as they navigate their remote care experience.
• Drive clinical decision-making utilizing guideline-directed medical therapy for heart failure along with evidence-based protocols for type 2 diabetes and hypertension.
• Proactively and asynchronously adjust medications based on patient vitals, symptoms, and trends.
• Supervise patients, manage clinical escalations, and work in collaboration with physicians or providers to enhance treatment plans.
• Manage a panel of patients receiving remote care services.
• Provide both synchronous and asynchronous care, which includes analyzing patient-generated health data, addressing remote monitoring alerts, engaging in secure messaging, and conducting scheduled virtual visits.
• Initiate and adjust guideline-directed medical therapy while collaborating with physicians, RNs, and care teams.
• Respond to abnormal vitals, symptoms, and lab results.
• Fulfill productivity and documentation requirements.
• Adapt to changing workflows, software updates, and operational processes.
• Utilize various technology platforms and offer feedback for the enhancement of clinical operations and technology.
• Collaborate with clinical and operational teams to enhance patient care, efficiency, and scalability.
• Contribute to the development of improved processes for a rapidly expanding digital health organization.
• Valid Compact Multi-State RN license, active and in good standing at the time of hire.
• Active NP licensure in TX, also active and in good standing at the time of hire.
• Over 5 years of experience in treating patients with chronic illnesses, including CHF, hypertension, and type 2 diabetes.
• More than 3 years of cardiology experience emphasizing heart failure management and guideline-directed medical therapy (GDMT).
• Current ANCC or AANP certification.
• Master’s Degree in Nursing as a Nurse Practitioner.
• Willingness to expand state licensure as Cadence introduces new markets and partners.
• Home office setup with consistently stable Wi-Fi and robust upload/download speeds.
• Completion of a Wi-Fi speed test is required during the interview process.
• Proven success in dynamic, fast-paced environments.
• Comfortable receiving regular feedback and adjusting processes accordingly.
• Highly organized with outstanding time management skills.
• Ability to balance patient care responsibilities with productivity expectations.
• Self-motivated and accountable while working remotely with minimal supervision.
• Experience in a CHF bridge clinic or T2D clinic, startup environment, and with remote patient monitoring technology is preferred.
• Proficient in using AI-assisted clinical tools such as Gemini, Open Evidence, or similar platforms.
• Clinical judgment must remain the primary factor guiding all patient decisions.
