Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver In-Home Health Assessments at members’ private residences

• Carry out approximately 45-minute visits that include reviewing medication and medical history, as well as performing physical evaluations

• Execute additional diagnostic tests as required by the member’s health plan, which may include diabetic eye exams, spirometry, and bone density screenings

• Utilize an iPad and user-friendly clinical workflow for each evaluation

• Interact with members individually, address health inquiries, and assist in closing care gaps

• Travel across the country to visit members and link them to suitable care

• Offer Signify Health services, encompassing Diagnostic & Preventive Services products

• Travel locally and within licensed states as necessary

• Conduct virtual visits when required

⛳️ Requirements

• Active, unrestricted license(s) in relevant coverage area(s)

• Board certification is mandatory

• Multi-state licenses are preferred

• Capability to safely perform duties in homes that may have household pets, secondhand smoke, and possible airborne allergens or irritants

• Willingness to travel locally and within licensed states as necessary

• Ability to conduct virtual visits as needed

• Qualification as a Nurse Practitioner or Physician Assistant
