Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Conduct health evaluations within the homes of members

• Perform visits lasting approximately 45 minutes

• Interact directly with members to address health inquiries

• Assess medications and review medical histories

• Execute physical assessments

• Carry out additional diagnostic tests as requested by the health plan, including diabetic eye exams and spirometry

• Utilize an iPad and follow a clinical workflow for each assessment

• Establish trustworthy relationships to enhance health outcomes

• Travel nationwide to connect individuals with appropriate healthcare services

• Undertake local travel and within licensed states as necessary

• Conduct virtual consultations when required

• Deliver Signify Health services, encompassing diagnostic and preventive care such as spirometry, diabetic eye exams, and bone density screenings

⛳️ Requirements

• Active, unrestricted license(s) in relevant coverage area(s)

• Board certification (mandatory)

• Multi-state licenses (desirable)

• Willingness to travel locally and within licensed states as necessary

• Capability to conduct virtual visits as required

• Ability to perform duties safely in homes that may have household pets, secondhand smoke, and airborne allergens or irritants

• Qualification as a Nurse Practitioner or Physician Assistant
