Nurse Practitioner, PA License Required

This is a fully remote position, open to applicants in United States.

📋 Description

• Address the care requirements for the Transitions of Care program.

• Execute telephonic outreach and arrange scheduled video or phone consultations for patients recently hospitalized and discharged.

• Conduct medication reconciliation and symptom assessments.

• Request necessary follow-up laboratory tests, medications, or supplies.

• Establish patient trust, clarify Galileo's care model, and link patients to services.

• Coordinate care among hospitals, emergency departments, pharmacies, and community providers.

• Collaborate with the nursing team.

• Achieve individual performance objectives and contribute to team-level population health initiatives.

• Deliver comprehensive transition-of-care follow-up within a 24/7 medical group utilizing 4-hour shifts.

⛳️ Requirements

• Certified Nurse Practitioner in Adult, Family, or Geriatrics.

• Active state licensure in PA, with the capability to obtain additional state licenses.

• Over 5 years of post-graduate clinical experience in Primary Care or Geriatrics.

• Two of those years may be replaced by experience in a Skilled Nursing facility or Long Term Care facility.

• Direct experience engaging with Medicare and Medicaid populations managing complex, comorbid conditions.

• Proficiency in motivational interviewing techniques.

• Comfort in a digital-first setting, with experience in or a keen interest in virtual care delivery.

• Strong technical skills.

• Exceptional written and verbal communication abilities.

• Availability to work 24-36 hours per week.

• Willingness to work weekends on a monthly basis.

• Bilingual in English and Spanish is a significant advantage, though not mandatory.

• Familiarity with Google Suite and Slack is beneficial, though not required.

• Active licenses in TX, NJ, IL, NY, MA, MN, LA, and/or SC are a plus.
