Nurse Practitioner, NJ License Required

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Address care requirements for the Transitions of Care program

• Conduct telephonic outreach and arrange video/phone consultations for patients who have recently been hospitalized or discharged

• Execute medication reconciliation and perform symptom assessments

• Request necessary follow-up labs, medications, or supplies

• Establish patient trust, clarify Galileo's care model, and link patients to relevant services

• Coordinate care among hospitals, emergency departments, pharmacies, and community healthcare providers

• Collaborate with the nursing team

• Achieve personal performance objectives and contribute to team-oriented population health initiatives

⛳️ Requirements

• Certified Nurse Practitioner specializing in Adult, Family, or Geriatrics

• Possession of active state licensure in NJ, with the capacity to obtain additional state licenses

• Bonus points for having active licenses in TX, PA, IL, NY, MA, MN, LA, and/or SC

• Over 5 years of post-graduate clinical experience in Primary Care or Geriatrics

• Up to 2 of those years may be substituted with experience at a Skilled-Nursing or Long Term Care facility

• Direct experience working with Medicare and Medicaid populations managing complex, comorbid conditions

• Proficient in motivational interviewing techniques

• Comfortable in a digital-first environment, with experience in or a strong interest in virtual care delivery

• Strong technological skills; familiarity with Google Suite and Slack is advantageous

• Exceptional written and verbal communication abilities

• Bilingual in English and Spanish, including cultural fluency, is a significant advantage

• Availability to work 24-36 hours per week

• Expected to work weekends on a monthly basis
