Nurse Practitioner – National After-Hours Team, Part Time

This is a fully remote position, open to applicants in Louisiana, +1 more state.

📋 Description

• Deliver empathetic telephonic care and clinical guidance to residents and facility personnel during evenings, nights, weekends, and holidays.

• Act as the primary source of after-hours assistance, offering medical guidance and care via phone.

• Utilize Curana’s telephonic platform for making and receiving calls.

• Facilitate care coordination among facilities, hospitals, and clinics.

• Provide cost-effective medical care addressing acute, chronic, and behavioral health issues in partnership with physicians and specialty providers.

• Conduct a minimum of 30% telehealth visits, which includes assessments of acute changes, falls, and controlled substance evaluations.

• Perform thorough assessments and accurately document encounters in the EMR, adhering to CMS requirements.

• Implement clinical protocols and practice guidelines to ensure safe treatment in place whenever feasible.

• Engage in mandatory education and training sessions.

• Maintain a fixed schedule of 24 hours per week: two six-hour shifts on weeknights and 12-hour shifts every other weekend on Saturday and Sunday.

• Provide necessary overnight and holiday coverage.

⛳️ Requirements

• Master’s Degree as a Nurse Practitioner.

• Valid, unrestricted licensure in Texas and Louisiana, or another approved state.

• National Nurse Practitioner certification as ANP, FNP, or GNP.

• Capability to obtain DEA licensure / Prescriptive Authority.

• Experience in managing both acute and chronic diseases.

• Clinical background in adult, family, or geriatric care.

• A minimum of 3 years’ experience as a Nurse Practitioner.

• Ability to acquire a collaborative practice agreement, if required in your state(s).

• Flexibility to work scheduled shifts in compliance with scheduling policies.

• Proficient in computer skills, including the ability to document medical information in written and electronic medical records.

• Experience working in a nursing home or with seniors in an acute care setting.

• Knowledge of geriatrics, chronic illness, and acute disease management.

• Familiarity with advanced illness and end-of-life discussions.

• Skill in developing and maintaining positive relationships with customers.

• Adaptability to change.

• Capable of managing high call volumes and conducting at least 30% telehealth visits.

• Ability to independently manage multiple calls while providing care across various states.

• Willingness to cover overnight and holiday shifts, including two holidays per year.
