Nurse Practitioner – National After-Hours Team

This is a fully remote position, open to applicants in Louisiana, +1 more state.

📋 Description

• Deliver compassionate telephonic care and clinical guidance during evenings, nights, weekends, and holidays.

• Act as the primary support for residents and facility personnel during off-hours, offering direction and medical assistance via phone.

• Utilize Curana’s telephonic platform to manage calls, facilitating care coordination between facilities, hospitals, and clinics.

• Provide high-quality, cost-effective care addressing acute, chronic, and behavioral health needs in partnership with physicians and specialty providers.

• Conduct a minimum of 30% telehealth visits, which include evaluating acute changes, falls, and controlled substance consultations.

• Handle high call volumes and manage multiple calls independently across various states.

• Execute comprehensive assessments and accurately record encounters in the EMR in line with CMS requirements.

• Implement Curana’s clinical protocols and practice guidelines to support treatment in place whenever feasible.

• Commit to 24 hours per week on a predetermined schedule.

• Cover two six-hour weeknight shifts and every other weekend Saturday and Sunday for 12-hour shifts.

• Provide necessary overnight and holiday coverage.

⛳️ Requirements

• Master's Degree as a Nurse Practitioner.

• Active, unrestricted licensure in Texas and Louisiana, or another approved state is essential.

• Additional active licenses in Tennessee, Mississippi, Arkansas, Alabama, Kentucky, Arizona, Colorado, Nevada, and New Mexico are highly preferred.

• National certification as a Nurse Practitioner (ANP, FNP, or GNP).

• Capability to obtain DEA licensure / Prescriptive Authority.

• Experience in managing acute and chronic diseases.

• Clinical background in adult, family, or geriatric care.

• A minimum of 3 years of experience as a Nurse Practitioner.

• Ability to establish a collaborative practice agreement, if applicable in your state(s).

• Capacity to work scheduled shifts in accordance with scheduling policies.

• Proficient in computer skills, including documenting medical information in both written and electronic medical records.

• Preferred experience in a nursing home or with seniors in an acute care setting.

• Knowledge of geriatrics, chronic illness, and acute disease management is preferred.

• Familiarity with advanced illness and end-of-life discussions is preferred.

• Strong ability to develop and maintain positive customer relationships.

• Adaptability to change.

• Must be available for overnight and holiday coverage.

• Two holidays per year are required.
