This is a fully remote position, open to applicants in Alabama, +4 more states.
• Deliver comprehensive remote virtual care management for serious illnesses to patients and their families in home-based, facility-based, inpatient hospice, palliative care, PACE, and various other service lines.
• Collaborate with medical personnel and interdisciplinary team members to ensure the highest quality of patient care.
• Conduct virtual evaluations of the needs of patients, families, and caregivers.
• Identify, manage, prescribe medications and equipment as necessary.
• Adapt and support personalized care plans in accordance with organizational policies and nursing practice standards.
• Evaluate caregiver capability and confidence, providing education when necessary.
• Respond to clinical inquiries, process orders and prescriptions, and assist with clinical needs of patients and families.
• Ensure access to electronic medical records (EMRs), e-prescribing systems, and required applications.
• Timely document interactions in the relevant EMR.
• Communicate and work in collaboration with primary teams to maintain continuity of care.
• Assist with discharge planning, including medications, discharge orders, and instructions.
• Engage in Quality Assessment and Performance Improvement (QAPI) initiatives, medical record evaluations, and peer reviews.
• Stay informed about current research and trends in hospice and palliative medicine.
• Take part in departmental and medical staff functions.
• Maintain communication with team members, patients, families, caregivers, providers, and staff while adhering to HIPAA regulations.
• Provide education to patients, families, and staff.
• Involve in the development and implementation of patient-centered care plans as part of an interdisciplinary team.
• Offer coverage and availability based on organizational requirements.
• Achieve expected productivity benchmarks and apply telemedicine billing processes where applicable.
• A minimum of 3 years of recent clinical experience, including significant exposure to hospice, palliative care, and/or acute inpatient care.
• A Master's or Doctor's degree in Nursing from an accredited Family or Adult Nurse Practitioner Program.
• National certification, licensure, and prescriptive authority for advanced nursing practice in the home state and any additional states where care is provided.
• Licensure and prescriptive authority must be obtained and sustained for all necessary jurisdictions.
• Applications for required licenses must be submitted within 60 days of hiring or notification.
• A current Basic Life Support certification from an approved organization by the hiring date.
• A Compact RN license in the home state by the date of hire.
• Willingness to work varying nights, evenings, weekends, and/or holiday shifts.
• Capability to function independently as well as collaboratively within a team.
• Exceptional interpersonal, collaborative, organizational, delegation, and follow-up abilities.
• Competence in managing multiple tasks and handling stressful situations.
• Comfort in working with terminally ill patients and their families in emotionally charged circumstances.
• Ability to thrive in a fast-paced, evolving, high-pressure environment while meeting deadlines.
• Proficient in utilizing patient health information systems, EMRs, e-prescribing systems, computer telephony, and other required applications.
• Proficiency with Microsoft Word, Excel, and PowerPoint.
• Advanced clinical judgment and problem-solving expertise.
• Capacity to maintain a HIPAA-compliant, temperature-controlled, and noise-free work environment.
• Reliable HIPAA-compliant high-speed internet connection of 100 Mb with a hardwired backup.
• Commitment to safeguarding patient privacy and using headsets and company-approved devices, websites, and applications.
• Agreement to undergo post-offer drug and/or alcohol testing and comply with the Drug-Free Workplace Policy.
