This is a fully remote position, open to applicants in Colorado.
• Assist complex patients in navigating the healthcare system.
• Provide holistic and integrated care to enhance the quality of life for kidney patients.
• Operate both independently and collaboratively with the healthcare team.
• Coordinate and ensure the delivery of quality, consistent, and cost-effective care.
• Conduct comprehensive health assessments.
• Manage complex patients with an emphasis on minimizing hospitalizations and optimizing health outcomes.
• Cover Pacific Standard Time (PST) and Mountain Standard Time (MST) time zones.
• Offer a flexible Monday to Friday schedule to accommodate patient and dialysis center needs.
• Deliver comprehensive care and manage complex chronic conditions for patients nationwide.
• Oversee patients in various stages, including early kidney disease, dialysis transition, kidney transplant, and life-sustaining dialysis.
• A strong passion for providing care to patients with complex, chronic illnesses.
• A minimum of two (2) years of experience as a Nurse Practitioner (NP).
• A current unrestricted NP license to practice in the relevant state.
• Current certification in Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS).
• Intermediate computer skills with proficiency in MS Word, Excel, and Outlook are required.
• A home office setup and high-speed internet connection.
• Willingness to obtain up to six additional state licenses for remote patient management.
• Spanish-speaking ability is preferred in certain markets.
• Preferred experience in Value-Based Care or Population Health.
• Experience in documenting and coding from a Medicare Advantage (MA) risk perspective.
• Experience conducting Health Risk Assessments on high-risk and chronically ill patients.
• 5+ years of experience as a Nurse Practitioner working with adult/gerontology populations is preferred.
• Demonstrated experience in the longitudinal management of chronic illnesses such as CHF, DM, CV disease, and HTN.
