Nurse Practitioner

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform virtual patient evaluations via video consultations

• Offer medication management services

• Create care plans and conduct patient follow-ups

• Collaborate with primary care providers, specialists, managed care organizations, and community partners

• Facilitate advance care planning and engage in goals-of-care discussions with patients and their families

• Assist in HCC coding, risk adjustment, and the closure of HEDIS and STAR quality gaps through documentation

• Take part in a shared rotating 24/7 after-hours triage system

⛳️ Requirements

• Active Nurse Practitioner license in Delaware

• Candidates must reside in Delaware or nearby states and hold a Delaware license

• Board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) is mandatory; general NP certification is insufficient

• At least 3 to 5 years of experience in independent Nurse Practitioner practice

• Experience in true primary care or long-term chronic disease management

• Significant and current experience in telehealth

• Capable of making clinical decisions based solely on video consultations without hands-on access, vital signs, or remote monitoring equipment

• Experience in value-based care or a clear understanding of the underlying philosophy

• Familiarity with HCC coding, HEDIS measures, and STAR ratings in everyday practice

• Active DEA registration with full prescriptive authority

• Preference for candidates with Medicare and Medicaid provider numbers

• Entrepreneurial mindset and adaptability to a startup environment

• This role must be the candidate's primary employment

• Candidates should not have multiple active employer commitments that conflict with a Monday through Friday, 8:00 AM to 5:00 PM schedule
