Nurse Navigator – FIND Program

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Facilitate communication and coordinate care among patients, primary and specialty care physicians, and hospital departments based on standardized care pathways.

• Ensure the availability, accessibility, and timeliness of services to enhance the quality of care, coordination, and patient experience.

• Evaluate, educate, coordinate, and refer patients with incidental radiological findings that require follow-up.

• Reach out to patients identified with incidental or unexpected findings and provide recommendations for follow-up.

• Deliver personalized nurse navigation interventions, assessing educational levels, learning barriers, and psychosocial factors.

• Ensure proper follow-up intervals while avoiding duplicate follow-up requests.

• Guide patients to the appropriate provider for recommended follow-up care.

• Assist primary care physicians in coordinating follow-up at intervals suggested by radiologists.

• Obtain additional imaging as specifically requested by primary care physicians.

• Provide education, navigation, referrals, and care coordination services.

• Track daily and periodic tasks using available software tools.

• Collaborate with the Incidental Finding program, Radiology, and Oncology colleagues to enhance the program and optimize patient care.

• Operate independently in a high-stress environment, identifying issues, proposing solutions, and participating in their resolution.

• Maintain compliance with relevant laws, regulations, Trinity Health policies, the Integrity and Compliance Program, and the Code of Conduct.

• Perform other assigned duties as necessary.

⛳️ Requirements

• Graduate of a recognized nursing school.

• Bachelor of Science Degree in Nursing.

• A minimum of three (3) to five (5) years of progressively responsible clinical nursing experience is required.

• Current license to practice as a Registered Nurse in the State of Michigan.

• Additional state-specific licensing may also be necessary.

• Strong problem-solving skills and a capacity to develop innovative solutions to challenges.

• Proven ability to effectively navigate various software applications.

• Excellent communication skills with Medical Staff, departmental personnel, Trinity Health department representatives, physicians, and other healthcare providers and facilities.

• Preferred knowledge of Radiology procedures, protocols, and processes.

• Familiarity with Windows-based programs.

• Strong organizational abilities.

• Proficient manual dexterity for operating a computer keyboard.

• Good hearing for participating in extensive telephone conferences and in-person communications.

• Ability to adapt to frequently changing work priorities, balancing the demands of the System Office and Regional Health Ministries.
