Nurse Navigator – FIND Program

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Facilitate effective communication and coordinate care among patients, primary care and specialty physicians, as well as hospital departments following standardized care pathways.

• Ensure the availability, accessibility, and promptness of services to enhance the quality of care, coordination, and patient experience.

• Conduct evaluations, provide education, coordinate care, and refer patients identified with incidental radiological findings that require follow-up.

• Reach out to patients with incidental or unexpected findings and recommend appropriate follow-up.

• Deliver personalized nurse navigation interventions, assessing educational levels, learning barriers, and psychosocial factors.

• Manage appropriate follow-up intervals and prevent duplicate follow-up requests.

• Guide patients to the right provider in accordance with follow-up recommendations.

• Assist Primary Care Physicians (PCPs) with timely follow-ups at intervals suggested by radiologists.

• Acquire additional imaging as specifically requested by the PCP.

• Educate and assist patients throughout the follow-up process.

• Provide referrals and education according to appropriate care standards.

• Utilize software programs to monitor daily and periodic assignments.

• Collaborate with local and regional colleagues, as well as Radiology and Oncology departments, to enhance the program and improve patient care.

• Identify issues, propose solutions, and engage in their resolution.

• Perform other assigned duties as necessary.

• Stay updated on relevant laws, regulations, Trinity Health compliance requirements, the Code of Conduct, and policies and procedures.

⛳️ Requirements

• Graduate from an accredited Nursing program.

• Bachelor of Science Degree in Nursing is required.

• A minimum of three (3) to five (5) years of progressive clinical nursing experience is necessary.

• Current licensure to practice as a Registered Nurse in the State of Michigan.

• Additional licensing specific to the state may also be required.

• Strong problem-solving skills with the ability to propose creative solutions.

• Proven capability to navigate various software applications proficiently.

• Effective communication skills to interact with Medical Staff, departmental personnel, Trinity Health representatives, physicians, and other healthcare providers and facilities.

• Preferred knowledge of Radiology procedures, protocols, and processes.

• Familiarity with Windows-based programs is advantageous.

• Strong organizational skills are essential.

• Ability to adapt to frequently changing work priorities and balance the demands of working with the System Office and Regional Health Ministries.

• Good manual dexterity for operating a computer keyboard.

• Adequate hearing for extensive telephone conference calls and face-to-face communication.
