Nurse Care Coordinator Lead, RN

This is a fully remote position, open to applicants in California.

📋 Description

• Recruit, hire, train, and onboard new team members.

• Provide direct supervision and performance management for assigned staff.

• Conduct regular one-on-one meetings, performance evaluations, and coaching sessions.

• Identify professional development needs and facilitate training and ongoing education.

• Address and document performance-related issues.

• Prepare staff schedules and manage coverage effectively.

• Maintain compliance with healthcare regulations, including HIPAA and state nursing boards.

• Ensure timely and precise documentation and record-keeping.

• Participate in quality improvement initiatives and process optimization efforts.

• Track and report key performance indicators and metrics.

• Coordinate with various departments to ensure smooth transitions in patient care.

• Maintain appropriate staffing ratios and coverage levels.

• Communicate with patients, families, and interdisciplinary team members.

• Participate in team meetings and clinical huddles.

• Act as a liaison between clinical staff and management.

• Report team performance, concerns, and achievements to leadership.

• Collaborate with community partners and external resources.

• Coordinate, schedule, and reschedule clinical appointments.

• Manage referrals to specialists and external clinical services.

• Follow up with families to minimize gaps in care and reduce no-show rates.

• Provide clinical updates to referring physicians.

• Coordinate lab work and documentation requirements with referring providers.

• Support providers for patients needing escalation of care.

• Provide evidence-based education to families regarding chronic condition management.

• Explain medication regimens, treatment plans, and care goals to patients and families.

• Respond to inquiries from patients and families via messaging and phone calls.

• Initiate and manage medication prior authorization requests.

• Communicate with pharmacies to resolve dispensing issues and coordinate refills.

• Track authorization status and escalate any delays.

• Maintain accurate medication records and document communications with pharmacies.

⛳️ Requirements

• Active California Registered Nurse (RN) license in good standing.

• Associate's, Bachelor's, or Master's degree in Nursing accepted.

• Experience in leading a team of clinical staff.

• Demonstrated proficiency with various technology platforms and electronic health record (EHR) systems.

• Strong organizational, communication, and interpersonal skills.

• Ability to manage competing priorities and collaborate in a remote, virtual environment.

• Comfort working with diverse, medically and socially complex pediatric populations.

• Enthusiasm for joining a rapidly growing startup and iterating on innovative care models.

• Reliable internet connection and a dedicated workspace suitable for remote work.

• Bilingual: English/Spanish.

• Preferred: Experience in pediatric, chronic care, or care coordination settings.

• Preferred: Familiarity with prior authorization processes and payer requirements.

• Preferred: Experience working in underserved or urban environments.

• Full-time availability, initially 40 hours per week.

• Availability Monday–Friday, 9 AM–6 PM PST, with a 1-hour daily break.

• Occasional Saturday coverage required.
