Medical Social Worker

This is a fully remote position, open to applicants in California.

📋 Description

• Direct the daily functions of the social services team, encompassing policies, procedures, and care coordination with other clinical providers.

• Recruit, onboard, and mentor social workers while providing clinical supervision, observing practices, and fostering professional development.

• Oversee the assessment, planning, and evaluation of social services care.

• Ensure that clinical documentation complies with state, federal, and ACHC standards.

• Collaborate with the Executive Director on the annual departmental budget and resource planning.

• Enhance knowledge of community resources and connect patients and referral sources to the appropriate support.

• Organize in-service training sessions and continuing education opportunities.

• Participate in quality improvement initiatives and identify strategies to enhance patient services.

• Execute additional duties as necessary for patient services.

⛳️ Requirements

• A Master of Social Work (MSW) degree from an accredited institution.

• A valid California license as a Licensed Clinical Social Worker (LCSW).

• A minimum of 3 years of management experience, ideally in home care.

• Strong understanding of healthcare regulations, ACHC standards, and business operations.

• Exceptional communication abilities and the capacity to establish trust with patients, families, staff, and the community.

• A self-motivated, organized, and adaptable approach—able to take initiative and collaborate effectively with others.
