Medical Social Worker

This is a fully remote position, open to applicants in California.

📋 Description

• Oversee the daily operations of our social services team, which includes managing policies, procedures, and coordinating care with other clinical providers.

• Recruit, onboard, and guide social workers—offering clinical supervision, monitoring practices, and fostering their professional development.

• Supervise the assessment, planning, and evaluation of social services care, ensuring that clinical documentation complies with state, federal, and ACHC standards.

• Collaborate with the Executive Director on the annual budget for the department and resource allocation.

• Develop a strong understanding of community resources to connect patients and referral sources with appropriate support.

• Organize in-service training and continuing education to maintain the team's skill set.

• Participate in quality improvement initiatives and assist in identifying innovative ways to enhance patient care.

• Perform additional duties as necessary for patient services.

⛳️ Requirements

• A Master's Degree in Social Work (MSW) from an accredited institution.

• A valid California license as a Licensed Clinical Social Worker (LCSW).

• A minimum of 3 years of management experience, ideally within home care.

• Comprehensive knowledge of healthcare regulations, ACHC standards, and business operations.

• Exceptional communication skills, with the ability to establish trust with patients, families, staff, and the community.

• A self-motivated, organized, and adaptable mindset—you take initiative and collaborate effectively with others.
