Medical Social Worker

This is a fully remote position, open to applicants in California.

📋 Description

• Oversee the daily functions of the social services team, ensuring adherence to policies, procedures, and care coordination with other clinical providers.

• Recruit, onboard, and mentor social workers, offering clinical supervision, monitoring practice, and facilitating professional development.

• Supervise the assessment, planning, and evaluation of social services care, ensuring clinical documentation complies with state, federal, and ACHC standards.

• Collaborate with the Executive Director on the annual departmental budget and resource allocation.

• Enhance knowledge of community resources and connect patients and referral sources to suitable support services.

• Organize in-service training and continuing education opportunities.

• Contribute to quality improvement efforts and identify strategies to enhance patient care.

• Perform additional responsibilities as necessary for patient services.

⛳️ Requirements

• A Master of Social Work (MSW) degree from an accredited institution.

• A valid California license as a Licensed Clinical Social Worker (LCSW).

• Minimum of 3 years of management experience, ideally in home care settings.

• Strong understanding of healthcare regulations, ACHC standards, and business operations.

• Excellent communication abilities and the capacity to foster trust with patients, families, staff, and the community.

• A self-motivated, organized, and adaptable approach—you take initiative and collaborate effectively with others.
