Medical Social Worker

This is a fully remote position, open to applicants in California.

📋 Description

• Direct the daily activities of the social services team, encompassing policies, procedures, and coordination of care with other clinical providers.

• Hire, onboard, and guide social workers, offering clinical supervision, observing their practices, and fostering professional development.

• Supervise the assessment, planning, and evaluation processes of social services care.

• Ensure that clinical documentation complies with state, federal, and ACHC requirements.

• Collaborate with the Executive Director on the annual budget for the department and resource allocation planning.

• Enhance knowledge of community resources and link patients and referral sources to suitable support services.

• Organize in-service training and continuing education opportunities.

• Participate in quality improvement initiatives and explore innovative methods to enhance patient care.

• Carry out additional responsibilities as needed to support patient services.

⛳️ Requirements

• A Master of Social Work (MSW) degree from an accredited institution.

• A valid California license as a Licensed Clinical Social Worker (LCSW).

• Minimum of 3 years of management experience, ideally in home care settings.

• Comprehensive understanding of healthcare regulations, ACHC standards, and business operations.

• Exceptional communication skills with the ability to cultivate trust among patients, families, staff, and the community.

• A proactive, organized, and adaptable mindset—able to take initiative and collaborate effectively with others.
