Medical Social Worker

This is a fully remote position, open to applicants in California.

📋 Description

• Manage the daily functions of the social services team, encompassing policies, procedures, and care coordination with other clinical providers.

• Recruit, onboard, and guide social workers, offering clinical supervision, observing their practice, and fostering their professional development.

• Supervise the assessment, planning, and evaluation of social services care, ensuring that clinical documentation complies with state, federal, and ACHC standards.

• Collaborate with the Executive Director on the annual budget for the department and resource allocation.

• Enhance knowledge of community resources and link patients and referral sources with suitable support.

• Organize in-service training and ongoing education opportunities.

• Engage in quality improvement initiatives and identify strategies to enhance patient care.

• Undertake additional duties as necessary for patient services.

⛳️ Requirements

• A Master of Social Work (MSW) degree from an accredited institution.

• A valid California license as an LCSW.

• At least 3 years of management experience, ideally within the home care sector.

• Strong understanding of healthcare regulations, ACHC standards, and business operations.

• Excellent communication abilities and the capability to build trust with patients, families, staff, and the community.

• A self-motivated, organized, and adaptable attitude—you take initiative and collaborate effectively with others.
