Medical Social Worker

This is a fully remote position, open to applicants in California.

📋 Description

• Direct the daily functions of the social services team, encompassing policies, procedures, and care coordination with other clinical professionals.

• Recruit, onboard, and mentor social workers, offering clinical supervision, observing their practice, and fostering their professional development.

• Supervise the assessment, planning, and evaluation of social services care.

• Ensure that clinical documentation complies with state, federal, and ACHC standards.

• Collaborate with the Executive Director on the annual departmental budget and resource allocation.

• Develop knowledge of community resources and link patients and referral sources to suitable support.

• Organize in-service training and continuing education opportunities.

• Participate in quality improvement initiatives and seek ways to enhance patient services.

• Carry out additional duties as required for patient services.

⛳️ Requirements

• A Master of Social Work (MSW) from an accredited institution.

• A valid California license as an LCSW.

• Minimum of 3 years of management experience, ideally in home care.

• Comprehensive understanding of healthcare regulations, ACHC standards, and business operations.

• Exceptional communication skills with the ability to establish trust with patients, families, staff, and the community.

• A self-motivated, organized, and adaptable approach—you take initiative and collaborate effectively with others.
