Medical Social Worker

This is a fully remote position, open to applicants in California.

📋 Description

• Direct the daily activities of the social services team, encompassing policies, procedures, and collaboration with other clinical providers.

• Recruit, onboard, and mentor social workers while providing clinical supervision, observing their practice, and facilitating their professional development.

• Supervise the assessment, planning, and evaluation of social service care, ensuring that clinical documentation adheres to state, federal, and ACHC standards.

• Collaborate with the Executive Director on the annual budget for the department and resource allocation.

• Enhance knowledge of community resources and connect patients and referral sources with suitable support.

• Organize in-service training and continuing education opportunities.

• Participate in quality improvement initiatives and identify strategies to enhance patient services.

• Carry out additional responsibilities as necessary for patient services.

⛳️ Requirements

• A Master of Social Work (MSW) degree from an accredited institution.

• An active California license as a Licensed Clinical Social Worker (LCSW).

• A minimum of 3 years of management experience, ideally within home care.

• Strong understanding of healthcare regulations, ACHC standards, and business operations.

• Exceptional communication skills with the ability to establish trust with patients, families, staff, and the community.

• A self-motivated, organized, and adaptable demeanor—you take initiative and collaborate effectively with others.
