This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Establish and nurture trusting, ongoing relationships with patients, their families, caregivers, and healthcare providers in both medical and behavioral health.

• Engage with high-risk or targeted patient populations through bi-directional communication to address their health-related inquiries, concerns, and care requirements.

• Identify and evaluate medical, behavioral, social, emotional, and financial needs to facilitate comprehensive whole-person care.

• Assist patients in understanding and adhering to care plans, health objectives, and preventive care requirements, including education and motivational interviewing.

• Conduct non-clinical assessments, surveys, and interventions to aid patients in navigating medical, behavioral, and social systems.

• Enhance the connection between patients and their healthcare providers by addressing obstacles, facilitating communication, and coordinating follow-up actions.

• Provide assistance with referrals, transportation, appointment scheduling, and linking patients to specialty care or community-based resources (CBOs).

• Leverage knowledge of referral processes, prior authorizations, care gaps, SDOH factors, and community programs to enhance patient outcomes.

• Appropriately escalate medical issues to RN Care Managers, Social Workers, PCP offices, or Guidehealth leadership.

• Timely and accurately document all interactions, assessments, and interventions in the electronic health record (EHR) and Guidehealth documentation systems.

• Exhibit proficiency in using various EHR platforms (e.g., Epic, Athena, Cerner), analytics tools, and Microsoft Office applications.

• Manage referrals, monitor tasks, complete reconciliations, and maintain comprehensive records in accordance with Guidehealth policies.

• Operate independently while closely collaborating with Guidehealth's case management teams and medical practice staff.

• Achieve assigned Key Performance Indicators (KPIs) related to patient engagement, health outcomes, and reduction of medical expenses.

• Participate in Guidehealth meetings, training sessions, and quality-of-care initiatives.

• Adhere to all HIPAA, PHI security, and Guidehealth privacy policies, while maintaining professionalism, compassion, empathy, cultural competence, and sound judgment in all interactions.


⛳️ Requirements

• Certified Medical Assistant (preferred) OR Certified Community Health Worker/Assistant OR Licensed Practical Nurse or a degree in a healthcare-related field with relevant experience OR Certified Emergency Medical Technician (preferred).

• Bachelor’s Degree in a health-related field (preferred).

• Patient Navigation certification is preferred or a willingness to obtain it within one year.

• Bilingual proficiency in English and Hindi is required.

• A minimum of 2–3 years of relevant experience in patient navigation, medical assisting, health promotion, care coordination, or similar roles.

• Equivalent experience may include several years in similar positions without formal certification.

• Experience in direct patient interactions in clinical or non-clinical environments, preferably with complex medical/social needs.

• Proven experience with EHR documentation and effectiveness in remote work settings.
